H3 is required by law to maintain the privacy of health information. We are dedicated to protecting client’s important and personal information.
HIPPA guidelines are strictly followed. A copy of the guideline is kept in the office for review as needed and offered to clients upon intake or at yearly review. A digital copy is also located in the Google Drive under H3 Policies.
A clinical record is maintained, which includes information such as the client’s diagnosis, treatment, progress, dates of services, and notes describing each therapy session.
All records are digital. Any non-digital paperwork is scanned into the HIPPA compliant electronic record system, and then shredded.
H3 does not release records without written consent, unless outlined below.
Issues discussed in therapy are generally legally protected as being both confidential and privileged. There are, however, legal limits to the privilege of confidentiality:
H3 utilizes an internet-based practice management program (EThomas) for billing, scheduling appointments, and reports.
Client at H3 are treated with respect. Abuse, harassment, exploitation or other such actions directed at clients is strictly forbidden.
Clients are encouraged to ask any questions they may have with regard to confidentiality and privacy issues.
