A core value of H3 is collaboration and the coordination of care between H3 clinicians, and other health care providers (HCPs) and services. These values are essential to increasing awareness and knowledge among HCPs and improving patient outcomes
Collaboration and coordination of care occurs in various ways, including the following:
After obtaining a signed release of information from the client, the clinician will formulate a coordination of care letter to the relevant HCP. This letter will include a list of the client's current diagnoses and the treatment plan. The HCP is encouraged to contact the clinician for clarification or with any concerns or questions. These letters are created on an as-needed basis, and will be mailed to the HCP.
H3 encourages the participation of relevant family members but only when appropriate, in the client’s best interest, and and when the client has consented and signed a release of information.
Within the limits of client confidentiality, every reasonable attempt is made to facilitate the collaboration and coordination of client care between H3 clinicians and other HCP's such as inpatient programs, partial hospitalization programs, support groups, Social Services, pastoral care, and others.
