H3 is required by law to maintain the privacy of health information. H3 complies with all HIPPA, State, and Federal laws and regulations.A medical record is maintained which includes information such as the patient’s condition, illness, treatment, progress, dates of service, and notes describing each therapy session.Medical records are stored digitally via Simple Practice.The office manager is responsible for overseeing the medical records and all related policies and procedures.Only authorized personnel have access to medical records, faxes, and computer generated reports and information. Authorized personnel include the Office Manager, Practice Director/Owner, Clinical Director, Billing Director, and Lead Therapist. Clinicians may only access the medical records of their own individual clients. The medical record may include documentation of clinician collaboration and/or supervision as applicable.The medical records includes a summary in the event that the client is discharges or transitioned to another service, unless the client leaves the service without notice.Documentation occurs at the time of service.All documents requiring disposal are shredded.No records may be destroyed in the event that they may be needed for legal proceedings, including those initiated against H3; subpoenas, investigations, ect.Medical records (current and closed) are reviewed routinely throughout the year in various ways. These reviews may include”●Case conferences●Audits●Informal reviews when an issue or need is identified.
