All H3 Contract Therapy providers are credentialed by insurance companies through H3, at the cost of H3. If a Clinician decides to terminate their contract and leave H3 before 12 months (1 year) of service, the Clinician is responsible for repaying half the cost of their credentialing ($500.00). Payment to be determined upon departure by Practice Director.For all clinicians who are actively seeing clients, H3 requires a notice of a minimumof 7 days (preferably30 days) for termination of contract. A request to terminate a contract must be in writing, and a plan for transferring clients must be included (see attached Exit Checklist).Due to the impact on the client, and H3’s professional branding, Clinicians are requiredto make contact with ALLclients informing them of their departure, and are expected to complete appropriate termination, as determined by the ethical requirements of their license. All communication from the clinician to the client must be approved by H3. Termination must be reflected in the progress note, or via chart note, for record purposes. For inactive clients, notifying client via US Mail is acceptable. All progress notes must be up to date at time of termination.
